I hereby give permission to The Seeing Eye, Inc., to use the child’s and likeness in photographs and/or video in all forms and media, whether now known or hereafter created, for any lawful purpose. I understand and agree that The Seeing Eye, Inc. has complete ownership of and full copyright to such photographs and/or video.
On behalf of myself and the minor child, I waive any right to royalty, fee, or other compensation by reason of such use. I also waive any right to inspect or approve the photograph, advertising copy, or printed or electronic matter in which the child’s name or likeness may appear..
On behalf of myself and the minor child, hereby RELEASE and forever discharge The Seeing Eye, Inc., its officers, trustees, employees, agents and representatives from any and all claims, liabilities, obligations, promises, agreements, disputes, demands, damages, causes of action or any nature, known or unknown, which I or anyone claiming on my behalf may have or claim to have against the foregoing releasees in connection with such use.
I understand that my electronic acceptance of this agreement is legally binding and has the same force and effect as a handwritten signature.
