By selecting “I Agree” and submitting this form, I certify that I am the parent or legal guardian of the minor child identified below and have the legal authority to provide this acknowledgment on the child’s behalf. I acknowledge that I have listened to, or had explained to me, The Seeing Eye’s Anti-Harassment & Standards of Conduct Policy, and I understand its contents and requirements.
I understand that it is the responsibility of the minor child while participating in The Seeing Eye program, to refrain from engaging in conduct that violates the Anti-Harassment & Standards of Conduct Policy. I further understand the importance of reporting conduct that I believe to be discriminatory, harassing, and/or retaliatory to the Senior Manager of the class, the Assistant Director of Training, or the Director of Training so that The Seeing Eye may take appropriate action.
I understand that any student who engages in conduct prohibited by the Anti-Harassment & Standards of Conduct Policy may be subject to disciplinary action, including possible expulsion from the program.
I further understand that my electronic acceptance of this agreement is legally binding and has the same force and effect as a handwritten signature.
